Please print and completely fill out these forms so we can expedite your first visit. Also please bring your medications, a list of your surgeries (with the years they were performed), your insurance cards and a photo I.D.

Please contact our office to verify acceptance of your insurance plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. As a courtsey to our patients, we will file your claim.

What To Expect

Being well-prepared for your appointment will ensure that Dr. Berlin has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. A thorough history and physical exam will be conducted, as well as a review of available test results. Recommendations will be made in regards to treatment plans and/or diagnostic tests. We look forward to your first visit.
